چکیده

Objectives This lecture will cover four areas: First, the early history of the PACU will be briefly discussed so that the reasons for the need and the development of a Post Anaesthetic Care Unit can be better understood. Second, the current guidelines and suggestions for the administration and care of patients in the PACU will be appraised. The third focus will be a review of epidemiological studies which have identified the commonest PACU problems. Specific strategies for prevention and treatment of common problems will be outlined. Since it is unlikely that current PACUs will escape costcutting strategies, the lecture will close with a brief discussion of future trends and potential changes in the organization and management of the PACU.

Maximizing operating room and recovery room capacity in an era of constrained resources.

HYPOTHESIS Three parallel processing operating rooms (ORs) (concurrent induction and turnover) with a dedicated 3-bed mini-recovery room (mini-postanesthesia care unit [PACU]) will optimize patient throughput and main PACU workload when compared with 4 traditional ORs or 4 parallel processing ORs. DESIGN Statistical and mathematical models projected the impact of parallel processing on case t...

Postoperative residual curarization from intermediate-acting neuromuscular blocking agents delays recovery room discharge.

BACKGROUND Postoperative residual curarization (PORC) [train-of-four ratio (T4/T1) <0.9] is associated with increased morbidity and may delay postoperative recovery room (PACU) discharge. We tested the hypothesis that postoperative T4/T1 <0.9 increases PACU length of stay. METHODS At admission to the PACU, neuromuscular transmission was assessed by acceleromyography (stimulation current: 30 m...

Fast-tracking (bypassing the PACU) does not reduce nursing workload after ambulatory surgery.

BACKGROUND Postoperative day-case patients are usually allowed to recover from anaesthesia in a postanaesthesia care unit (PACU) before transfer back to the day surgical unit (DSU). Bypassing the PACU can decrease recovery time after day surgery. Cost savings may result from a reduced nursing workload associated with the decreased recovery time. This study was designed to evaluate the effects o...

A pilot study of recovery room bypass ("fast-track protocol") in a community hospital.

PURPOSE To evaluate the effectiveness of short-acting anesthetic drugs and techniques to achieve recovery room bypass criteria after minor surgery in a community hospital environment. METHODS After agreement by a multidisciplinary committee, a pilot project was undertaken to assess the usefulness of ultra- short acting anesthetic drugs and pre-emptive analgesia to facilitate rapid recovery fr...

A simplified scoring system for the post-operative recovery room.

A new and very simple scoring system for recording post-anaesthetic recovery is described. When numerical values are assigned to factors indicating recovery progress or lack of it is obvious. The records may have medico-legal value. It can be incorporated into the Recovery Room Record.
